An original and a copy document bearing the Seal of Singapore's Notary Public

A copy of every page of the original documents

Process fee of S$ 30.- per document

An application form which can be downloaded from here

Submission Hours: Monday, Tuesday, Wednesday, Thursday (except Embassy's holidays) from 9.30 - 11.30 hrs.
Remarks:
- In case of not submitting the owner’s document, the representative must bring the following:
- An authorization letter to a person that applied on behalf of the document owner
- A photocopy of the Identity Cards of the document owner
